Community engagement teams
Community Engagement Teams (CET):
The Community Engagement Team (CET) is intended to assist individuals and their families cope with the effects of mental illness and substance abuse disorders while avoiding the criminal justice system and emergency mental health systems whenever possible. The services includes an individualized, recovery-focused approach that promotes wellness, self-management, personal recovery, natural supports, coping skills, self-advocacy and the development of independent living skills.
The program consists of Licensed Clinicians, Case Managers, Comprehensive Community Support Workers (CCSS) and Certified Peer Support Specialist (CPSS) to provide onsite engagement with individuals in need of CET services. The goal reduce the number of emergency room visits, incarceration and recidivism. Mental health and behavioral health services and referrals will also be available for family members.
Referrals come from outreach events and services where our staff is active within the community.
Outpatient/Step Down
Continue with the evidence based Matrix Model aftercare up to one (1) year after completion of the Intensive Outpatient Program, as clinically indicated, to include:
Aftercare/Support Group one (1) time per week for ninety (90) minutes for weeks17 to 52.
Attendance at twelve-step or other approved outside support groups two (2) times per week for weeks 17- 52.
Provide an exit interview (follow-up) session with each client upon program completion/termination.
Provide extended case monitoring/enhanced follow up post treatment via telephone interviews at intervals up to one (1) year after program completion/termination.
Provide program compliance information of attendance to individual sessions and groups to Community Custody personnel as well as case coordination as needed.
Outpatient Substance Abuse Treatment
New Mexico Hope provides evidence-based intensive outpatient substance abuse treatment New Mexico residents. We utilize the Matrix Model intensive outpatient program (IOP) which lasts for sixteen weeks. The IOP provides nine (9) hours of treatment weekly to include individual, groups and outside support group involvement for the first month and up to seven (7) hours of treatment per week for the following three months after the individual completes the Early Recovery Group during the first month of program involvement. Each individual will have a comprehensive assessment (Behavioral Addictions Severity Index) and a follow up reassessment at the time of program completion/termination.
Community treatment interventions are initiated with a comprehensive assessment (AccuCare-Behavioral Addictions Severity Index Assessment) completed by a clinician that will identify specific needs and goals for each individual in the program. Individual and/or Con-joint counseling sessions are interspersed throughout the IOP depending on individual need. The individual and conjoint support is designed to provide additional and continuing support with abstinence as well as monitoring of attendance and participation with outside support groups. These sessions provide strengthened support with personal relapse prevention planning.
During the first month clients attend the Early Recovery Group which addresses issues common with individuals who have just begun a period of abstinence. Keeping track of time sober and clean and learning how to schedule time are introduced during this group cycle and then continued throughout the duration of the IOP as clients begin to shift external control to internal control and individual responsibility.
Individuals attend the Relapse Prevention Groups for the entire four months to learn about their addictions and substance abuse and how to change their lives in order to live sober. These groups are based in cognitive behavioral therapy (CBT) and provide each person with tools to assess personal situations and ways to change their thoughts, emotions and behaviors. This provides each client with a personal recovery toolkit. Each client is given a workbook for the course of their treatment and can be continued when they have completed the program. This program is abstinence based and includes information about the twelve step programs to enable individuals to learn how and why those programs can be sources of positive support for sustained recovery.
The Family Education Group encourages individuals to have family members and significant others attend group therapy. Each group provides education about substances of abuse and dependence as well as the recovery process itself. Time is provided for discussion to assist individuals in understanding both the addiction and the recovery processes. Individuals attend this group weekly for the first three months. After completion of the Family Education Group, clients attend a Support Group for the following and last four weeks of the IOP. This provides continued support as well as a bridge to aftercare which is critical to the recovery process. For individuals who are in Community Custody for an extended period up to one year, the ongoing Support/Aftercare Group provides continuing treatment and support. This group is also offered to clients who wish to continue to attend voluntarily to assist them in a sustained recovery process.
The Matrix Model includes drug screening as an integral part of substance abuse treatment. As CCP provides extensive and ongoing drug screens and daily monitoring with the sobrietor, New Mexico Hope will continue to work with the agency referrals to obtain drug screening results through case coordination with clients’ trackers.
Twelve-step and other support group involvement will be monitored and assessed continuously through review of get well card program attendance verifications as well as self-reporting in groups by individuals on a regular basis. Due to group content supporting twelve steps, there are regular opportunities to assess individuals’ program participation through group discussion.New Mexico Hope will also include a Peer Advocate Program. Individuals will be selected to assist with groups who have at least six months of sustained recovery and who are interested in voluntarily being trained and participating as a group co-leader and/or group facilitator.
Our program goal is to support court-mandated clients and self referrals in gaining and maintaining abstinence and provide concurrent recovery monitoring while participating in the program and enhanced/follow-up monitoring after program completion to support sustained recovery and help prevent relapse.
All treatment provided will adhere, as applicable, to all provisions of 42 CFR Part 2 (Confidentiality of Alcohol and Drug Abuse Patient Records; Final Rule) and the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), the Standards for Privacy of Individually Identifiable Health Information 45 CFR Sections 160 and 164, and any other requirements for release of related information collected.
Compliance Monitor/Tracker Liaison
Each client will be expected to attend all scheduled assessments appointments, individuals therapy sessions, treatment groups, family education groups, and any other treatment/aftercare related activities clinically indicated by the assigned therapist and treatment team which will be tracked by our Compliance Monitor. Each client will be assigned a scan tag which will consist of the client scanning their tag reflecting their assigned client number for any time they are on the premises. Additionally, this will also include each client signing in for each session and our CM will also be able to ensure compliance and attendance through visual checks through our security camera system and one-way security windows attached to each group room. We will ensure reporting to respective County Community Trackers, Probation, Parole, and any other qualified agency within 30-60 minutes of non-compliance or the client missing a treatment related activity (as appropriate and negotiated with the referring agency). This will include submittal of any computer based or paper based reporting protocols established with each referring agency.
Case Management
The comprehensive case management component is the link to community resources when a client initially enters the treatment program. This link will assess clients for eligibility for Medicaid, housing, TANF, and other community based services. The service will also serve as the follow-up mechanism for clients who also discharge from the treatment program in order to ensure the linked community service was followed through with by both the client and the referred to agency. This service will also assess for recidivism, client satisfaction with the treatment services provided to the client.
